To the bottom left of this page is the official nomination form for the "Alumni of the Year" awards.

 

You, the nominator, will input YOUR information for the boxes: NAME, and EMAIL. Please provide your first and last name, and a valid email address (these will be used for communication purposes only).

In the SUBJECT box, please write the name of the nominee, and if possible, the nominee's graduating class.

Ex. John Doe, Class of 2002.

   

 In the MESSAGE box, you must include all of the following:

         -nominee's current contact info.

          (phone number and/or email)

         -reason for nomination

         -brief sentence of relationship with nominee
